What are the documents required to apply for duplicate DMC in PTU?

You can do it online via an online portal or you can visit the campus for the same. This is the only option that you have. The documents that you need to carry with yourself are as follows:

• Diploma/Degree Certificate
• Attested copy of Matriculation certificate
• NOC from concerned college
• Original copy of First Information Report (FIR)
• Copy of requested certificate/Result Notification
• ID proof 

Along with this, ask them to give you a written declaration stating that you are a student of the college and have cleared all the subjects of the semester and that you require a duplicate DMC.
